Solve the ORDERS IS ORDERS Cryptogram

Anybody can enter the contest once and the only criteria to get entered is to crack the cryptogram below and filling out the form with the correct answers. The entry deadline is 15 May 2009.

Marine Gunnery Sergeant James Mitchell and Private First Class Spivits are sent on a treacherous mission behind enemy lines to reach the American consulate in the Chinese city of Shunkien—which is under heavy assault by invading Japanese forces.
Mitchell and Spivits have been given an encoded message for you to decipher—the item named in the message is what they need to get to the consulate and it is a life and death matter!
To untangle the code, you need to write down the letters in large block letters, then underneath this, write down the same sequence of letters in reverse starting from the end of the line. The last step is to replace each letter with the letter that appears two spaces to the left of that letter in the alphabet.
